What you will still need outside this format:
- The employer's own notice of lock-out in Form XII: the register and the intimation cover it, but the notice itself is not drafted.
- The application to the Tribunal in Form II after conciliation fails, and the pleadings before the Tribunal.
- A voluntary arbitration agreement under section 42 (Form V).
- Stamp duty on the affidavit filed with the conciliation reply, under the State Stamp Act, and its attestation.
- The State's rules and forms where the State Government is the appropriate Government, and any State law on recognition of unions.
- The wage-structure computation behind a settlement: use the Wage-base determination working paper in the Structuring pay under the 50% rule pack.
- Advice on whether a particular strike or lock-out is illegal, and on any disciplinary action or wage deduction arising from it.

The law in brief. Section 62 now applies to every industrial establishment, not only public utility services. No worker may go on strike in breach of contract, and no employer may lock out, (a) without notice within sixty days before the strike or lock-out, (b) within fourteen days of the notice, (c) before the date given in the notice, (d) during conciliation and seven days after it concludes, (e) during proceedings before a Tribunal or National Industrial Tribunal and sixty days after, (f) during arbitration under a section 42(5) notification and sixty days after, or (g) while a settlement or award on the same matters is in operation. "Strike" includes concerted casual leave on a given day by fifty per cent or more of the workers (section 2(zk)). A strike or lock-out in breach of section 62 is illegal (section 63).
The Central rules. A strike notice is in Form XI, signed by the secretary of the registered union or, where there is none, by five elected representatives of the workers, with copies to the conciliation officer, the Chief Labour Commissioner (Central) and the Secretary, Ministry of Labour and Employment; its date of receipt is the reference date for section 62(1)(a) (rule 25). A lock-out notice is in Form XII, given to the secretary of every registered union, copied to the same three authorities and displayed at the main entrance (rule 26). The employer must report every strike notice received, and every lock-out notice given, electronically to the conciliation officer and the Chief Labour Commissioner (Central) within five days (section 62 and rules 25(3) and 26(3)). Where a strike or lock-out already exists, no notice is needed, but the employer must intimate it on the day it is declared. In the State sphere, the State's rules decide the form, the authorities and the mode: check them and adjust Parts C and D.
