Performance Management
The documents HR needs to manage performance fairly and defensibly: an annual appraisal record with weighted goals, calibration and an increment workbook in Excel, a performance improvement plan with fortnightly reviews and outcome letters, and the letters of caution and final warning, each marking where the law requires a different route.

ANNUAL PERFORMANCE APPRAISAL FORM AND RECORD
Appraisal cycle [Appraisal cycle, e.g. 2026-27] · Review period [Start date of the review period] to [End date of the review period]
How to use this form. Parts A, C and D are completed and signed at the start of the cycle, when goals and weights are agreed; Part F at the mid-year review; Parts G to L at the year end. The Excel version computes the weighted results in Parts C, D and E and the increment and variable pay in Part K; in Word, compute them by hand using the method stated under each table. A rating in this form is a managerial assessment of performance, not a finding of misconduct: where conduct is in issue use the letter of caution and final warning or the charge sheet, and where the year-end rating is 2 or below consider the performance improvement plan in this pack.
