RECORDS INDEX AND RECONSTRUCTION REGISTER

Custodian of the records: [Name of the custodian of records], [Designation of the custodian]
Reconstruction approved by the Board on: [Date of the Board meeting] · Register opened on: [Date the register was opened]

This register does three things. Part A indexes the Company's records file, volume by volume, and shows for each record whether what the Company now holds is the original, a reconstruction or only copies. Part B logs every entry made in a reconstructed register, with the evidence relied on, so that any entry can be traced back to its source. Parts C and D give the certificate placed at the front of each reconstructed register and the register of copies obtained from outside sources. Part E is the sign-off.
