AFFIDAVIT
[Rule 37(1)(b) of the Limited Liability Partnership Rules, 2009]
I, [Full name], son/daughter of [Deponent's father's or mother's name], aged about [Deponent's age (years)] years, residing at [Full address], holding PAN [Deponent's PAN] and DPIN [Deponent's DPIN], being a Designated Partner of [LLP's name] LLP, LLPIN [LLP's LLPIN], having its registered office at [LLP's registered office address] (the "LLP"), do hereby solemnly affirm and declare as follows:
1. That the LLP was incorporated on [LLP's date of incorporation] under the Limited Liability Partnership Act, 2008.
