[Name of the provider], a company incorporated under the Companies Act [2013 / 1956], having its registered office at [Provider's registered office address], bearing Corporate Identity Number [CIN], Permanent Account Number [Provider's PAN] and Goods and Services Tax Identification Number [Provider's GSTIN], acting through [Provider's authorised signatory's name], [Provider's authorised signatory's designation], duly authorised by a resolution of its Board of Directors passed on [Date of the Provider's board resolution], hereinafter referred to as the “Provider”, of the ONE PART;
