MASTER DOCUMENT REQUEST LIST FOR A NEW CLIENT
Client: [Name of the client] · PAN: [PAN of the client] · CIN, LLPIN or registration number: [Registration number of the client]
Services engaged: [Services engaged, as marked in the scope schedule] · Engagement letter dated: [Date of the engagement letter]
Request issued on: [Date of the request] · Firm contact: [Name, e-mail and mobile of the firm contact] · Client owner of this list: [Name and designation of the client owner]
Send this list on the day the engagement letter is signed. Delete the Parts that do not apply to the services engaged (for example Part H for a client without payroll). It overlaps deliberately with the onboarding forms already in the catalogue, so use it as the single list the client works from: the know-your-client particulars are captured on the Client onboarding and know-your-client form (FV04-L9), the master data on the Client master sheet (FV06-L2), and the recurring monthly data through the Period records request (FV06-B4) and the GST monthly data request (FV05-K2). Update the Status column as documents arrive and keep the list on the permanent file.
How to use this list
To take over your accounting, tax and compliance work we need the documents and information below. Please:
send each item through [Name of the secure upload channel], in the form stated, by the date given for its Part. Part J (pending notices) is needed within two working days, because the time for replying to a notice runs from the day it was communicated to you;
mark an item Not applicable if it does not exist for you. That is itself information we rely on, so please do not leave an item blank;
not send any password, PIN or one-time password by e-mail or messaging app. Part I lists the access arrangements we will set up with you instead;
send copies, not originals. Original books, registers and title documents stay with you;
tell us if anything listed has been lost, is with a previous adviser, or is held by a bank or lender.
Status codes used below: Received, Partly received (we will tell you what is missing), Awaited, Not applicable.
